ResponsibilitiesTo attend any new patient (admission) within prescribed time frame To examine & take a proper history of the patient (If he is in a position to give it on his own) or from his relative. Check any previous records, if available. Charts down all the details in the history sheet. Inform the respective consultant about the admission follow his orders. Instruct the nursing staff about the necessary orders to be carried out. If there’s a transfer in-patient from the ICU. Read the transfer summary/ clinical notes carefully and act accordingly. Inform the respective consultants whenever seems significant. Shift Duty: To take rounds and examine individual patients at least once in every shift and as & when needed. Inform the respective consultant/ Registrars if any new findings. To put the notes in clinical sheet. To enter the relevant details of each and every patient accordingly. Attend rounds with the respective consultants and carry out the necessary changes in the order. Enter them in the treatment sheet. Fill the Medicine Card and Non Drug Order sheet To instruct the nursing staff about orders and follow them up To follow up with all the investigations to be seen and trace their results. Note down investigation results in the investigation sheet. To Document Verbal Orders correctly in clinical notes if given by consultants. To respond to emergency calls and to activate code blue if any medical emergency is encountered beyond your control. To identify the critical status of patient and shift him to critical care area in time. Always contact primary consultant if found necessary at any point of time.Qualification & Experience   Able to work accurately and with minimal supervision. Ability to comprehend written instructions given by the Doctors and the other related departmental personnel. Ability to plan & organize ones work schedule effectively. Speaking and listening are essential requirements to understand and carry out the instructions given by the supervisors and other related departmental personnel. Affinity to work in team. Verbal Ability/Communication Skill: Ability to comprehend verbal instructions given by the supervisor or head of the department. To give accurate and tactful explanations to patients. Problem solving/ Decision-making To think independently and exercise own judgment in consensus with the supervisor. Enthusiasm. Open-mindedness.

ResponsibilitiesProvide comprehensive patient care within the assigned specialty/department.. Conduct patient assessments, formulate management plans, and ensure appropriate follow-up. Assist in procedures and interventions as required by the department. Manage routine and emergency clinical cases effectively. Act as the primary liaison between junior doctors (JR/Residents) and Senior Consultants Ensure accurate and timely communication of patient status, case updates, and clinical concerns. Escalate complex or high-risk cases to senior consultants promptly. Facilitate structured handovers and maintain continuity of care. Coordinate OT schedules, case prioritization, and resource allocation in consultation with seniors. Take a leadership role during emergencies and code situations. Coordinate rapid response with consultants and multidisciplinary teams. Ensure adherence to departmental protocols and clinical standards.

ResponsibilitiesRegular ward rounds & patient interaction of all In Patients. Initial Assessment and Daily assessment in regard to Diet and Nutrition. Taking part in IDTR. Prescribing diets for all patients using the principles of nutrition & diet therapy. Responsible for interfacing the functions of diet dept with F & B services. Providing Inputs to F & B and ensure that proper diet is served to patients as per plan. Providing appropriate therapeutic & non-therapeutic diets to patients within their socio cultural constraints. Planning & providing special diets for patients with special nutritional needs. Helping in Preparation of any special recipes if required for patients. Diet consultation for Inpatients on discharge, providing necessary inputs to be followed at home. Diet consultation for OPD & Health Check Ups where deemed necessary. Prepare written nutrition education materials. Keep abreast of relevant nutrition research changes, changes in delivery of nutrition services etc. Participate in appropriate nutrition health education programs. Provide input in planning of nutrition services activities. Ensure quality control & implementation of standards as for safety, hygiene Participating in quality improvement programs and relevant operational activities as per directed by Management.
